Capacity Assessments from a Designated Capacity Assessor can be useful to determine whether someone is capable of managing their property, or of granting or revoking a power of attorney for property. This type of assessment does not assess a persons capacity to make health care decisions, nor to accept/decline admission to long-term care. Various pieces of legislation indicate a need for or may benefit from an assessment that is not part of the SDA (e.g. testamentary capacity, capacity to instruct counsel, etc.). An opinion is formulated based on the appropriate legislation and jurisprudence, an interview, and then documented in a letterhead. Based on the purpose, the letter may also be presented to the courts and there may be a need for court appearances. It is recommended you consult a lawyer prior to requesting a letter of opinion. At this time, I do not complete Letters of Opinion for personal care. 

Bio-psycho-social-cultural Assessment...................................................... $175

Bio-psycho-social-cultural Assessments are a holistic tool utilized by social workers to better understand client experience, and therefore to better understand treatment needs. If you are feeling unclear about treatment goals, are looking for referrals beyond counselling, and/or just want to try a new way of understanding your situation, this service could be for you. Clients will be provided with an assessment report that includes service recommendations, including recommendations for referrals to outside services such as psychiatrists, psychologists, local social and health services, etc., if appropriate. External referrals are completed at no extra cost should you desire to proceed. 

Montreal Cognitive Assessment (MoCA)...................................................... $80

The MoCA (Montreal Cognitive Assessment) is a validated, highly sensitive tool for early detection of mild cognitive impairment (MCI) in hundreds of peer-reviewed studies since 2000, and is in use in many clinical settings globally. The MoCA is not diagnostic in nature, and is used to identify clients who need further assessment by a psychologist/physician/psychiatrist. Those concerned about cognitive impairment/decline may benefit from a screening with the MoCA to a) establish a baseline score and monitor changes over time, and/or b) identify need and seek referral for further assessment. Clients whose score indicates a need for further assessment can be referred to an external clinician at no extra cost. 

Intake Assessment for Counselling...................................................... $125

The importance of a good assessment for counselling cannot be overstated. This service ensures client safety through getting as much information as possible before treatment, and results in setting realistic, and relevant goals for counselling. This service can take between 45-minutes to 2-hours depending on the complexity of the situation, and is charged at a flat-rate. Clients will be provided with an assessment report that includes recommendations for treatment, and a treatment plan should they elect to proceed with counselling.

Individual Counselling (In-person)......................................................$160/hr.

Individual Counselling (Secure Virtual Platform)......................................................$125/hr.

Group Counselling......................................................$100/hr. per person; Group Counselling for a specific agency may be charged a lower cost
